I paid for my daughter's braces and every appointment was for 9 months to 1 year later. Last month after I requested they be removed because I'm not willing to get main teeth pulled and no services being performed.
They never touched her bottom teeth throughout the years. Every appointment was a confrontation, Rubber bands or few services or suggestions for regular dentistry. Regular dentistry do other services instead. Charges and no service.
They refused to remove the braces 4 wks ago unless I sign a waiver after their negligence. They then called the police on me and now refuses to give me her medical records.
Mesquite office is very rude and unpredictable. They surrounded me accusing me of threat, told me to go elsewhere and now won't give me the medical records.

I was charged $4616 by Monarch Dental in Weatherford, TX. I am disabled on Medicare Advantage Plan.
When I questioned the office, that I paid, about submitting my claim for services I was told a claim would not be submitted because the charges are what my insurance requires me to pay. I questioned this more than once and got the same answer. I was finally able to speak to a knowledge insurer representative who indicated my responsibility was $2330. The office near me also gave me an Intellisonic electric toothbrush as a gift.
Its an awesome toothbrush that I discovered I was charged $105 for this gift. I am very disappointed by this interaction with Monarch Dental. It seems quite unethical to be told more than once they did not file on my insurance. The insurance representative at the Weatherford office is named Angela.
This is extremely upsetting.
This is a horrible situation I find myself in. Customers should not be treated this way.

Teeth came out of my dentures
I had my new dentures made a year ago. I asked them before I left if I was all settled with my bill the told me yes.
Now I get a bill for three hundred and ninety five dollars. But. Thats not all one month after that I had two of my teeth come off my dentures. I called to see if any kind of warranty was on them and they told me no.
Paid over three thousand dollars for these teeth but they did not even make it a year. Needless to say they can keep sending me that bill and not going to pay it.

Terrible experience and crooks
I would like to give my friends a heads up, DONT GO TO MONARCH DENTAL in Weatherford !!not only was it awful but they up charged everything on our insurance ! They refused to do my regular cleaning because they said I needed a deep cleaning ( I get my regular check up and cleaning every 6 months and have never been told I needed a $218 cleaning ) look at Google reviews on them, and the insurance reviews, they are complete Scammers !!
Terrible experience! Do NOT recommend, especially for anyone with kids.Our daughter is 14 and was very nervous about getting injections to numb her for fillings and wanted me to go back with her, at least until the numbing was done. The person who called her back, stopped me from going back with her and told me she would come and get me BEFORE they started the injections, I waited, then another lady came out and said the Dr. has already started to get her numb and she is doing fine when I asked is she ok with that ?
She assured me they she was and was doing fine. In the mean time my daughter started texting me, asking why they hadnt brought me back and that they had already done part of the injections but that they werent able to get her numb yet. This was the point I went to talk to the office e manager, I was LIVID ! They took a minor away from the parent and didnt allow me back with her!
Very unacceptable! I was taken back at this point and my daughter was visibly upset and the second woman stuck her head in and stated that she didnt know she wanted me back there,( with a attitude I told her that it wasnt our problem that they didnt communicate with one another and that was definitely not what was best for my daughter! She walked off, the office manager came back in and told us at this point they were stopping the appointment and all our future appointments would be canceled because it wasnt ok for me to be back there cussing at them ( I never said one cuss word when I was back with her ) she basically kicked us out of office,(she actualy, took the bib off her and told her to give her the glasses and that this appointment was OVER and she would cancel all of our scheduled appointments to come ) so my daughter got 5 shots in her mouth and we had to go before her app to get her brace wires cut off and back after to have them put back on, all for NOTHING !! The dentist came
Out of the office after us to apologize and he was unaware of any of this and had no idea that the First woman had stopped me from going back with her, had agreed that that should NEVER happen with a minor child.
He was nice and apologetic but the office manager and the second woman were hateful and very UNPROFESSIONAL !!! The office manager even tried to say that my daughter had said she was fine without me back there ( she wasnt even back there, so has no idea ) my daughter kinda laughed out loud and said I did not say that, no one even asked me or gave me a chance to say anything before they started doing it ! We will never go back !
I would have given zero stars if I could have, terrible experience for my daughter, wasted time 5 shots in her mouth and she still has 4 fillings that she needs!!

Request a $60 refund on the 14th of this month. Called back on the 24th because I still hadnt received my refund they said they say when I called and requested it but the office never submitted it!! They had to do it all over again and!

On or about May 24, I received a filling from Monarch Dentistry. Then in July, the filling fell out, so they replaced the filling in August.
That replacement fell out in October, and now they're telling me I need a crown.
I don't have the money for a crown. I live on a fixed income, so I just want my refund for the filling because I've switched dentistry.

Monarch Dentistry performed a filling for me back in May. The filling fell out in July, so they refilled the tooth.
Then the second filling fell out in September. Now they're telling me that I need a crown.
I don't have the money for a crown, so what am I to do with the money I spent for the filling? Can someone reimburse me for the filling, because I have a new dentist.

I do NOT recommend them. First of all, any dentist, or doctor, that walks out on me, basically while I'm still telling him something is not a good one.
Maybe it's something he doesn't want to deal with, but you just don't walk out on your patients like that. He is always in a huge hurry. Probably to see more patients each day = $$$. It would take too long to explain it all but suffice it to say negligence at the very least and assumptions they shouldn't make.
RUDE dentist and staff, and pretty ignorant on MEDICAL ISSUES. NO interest in patient satisfaction. I was an RN for years, prior to being disabled with chronic fatigue/ME so I'm a lot less likely to cower because they're being *** but I just want people to think about it. I was treated very badly for basically standing up for myself and pointing out the reason I had these giant cavities that hurt so badly.
Their negligence. Because after he told me two teeth would need to be filled shortly, long ago, when they started showing signs of cavities, I called and the staff told me nope there is nothing on your chart about that. That is pretty negligent to me and caused this whole issue.
So I also recommend that they look very hard at their charting, I know of what I speak. Maybe many patients love them, but I think folks have a right to know the bad too .
Also, he clearly treated me like a drug seeker because he does not take time to LISTEN and is ill-educated on medical issues.

Company fixed the issue and I have been provided with apology. I called them several times and filed a complaint with Better Business Bureau.
Up to Nov 27th, 2023, they have only performed the following procedures:
One extraction
One root canal
Three crowns
Only one crown was done right, and the other two were done too short.
On March 13, 2024, I had to return to t the Bountiful office because one of the crowns (I believe #28) fell off. The dentist rebuilt the base and ordered another crown.
On April 1st, I returned so the crown could be installed. They had a different dentist (a Latino from Bolivia), he had difficulty working on the built-up base and destroyed it and had to extract the tooth. After extracting he used a suture thread that was so long that over 12" of it was resting on my chest. He didn't do a good job on the 2 stitches.
Now I have *** in my gum partially closed by the flab of the outside part of the gum that is loose and the adjacent tooth has now a partially exposed root.
Their manager told me they would refund me $478.00 that was part of the insurance that didn't cover it. They never refunded me and they kept the portion of the money my insurance paid them.
Their new manager advised me not to go back there since she was not confident their new dentist was experienced enough to fix my problem.

Awful and unwilling to rectify the situation
This notice was surprising, given my understanding that all accounts related to Naomi's orthodontic treatments were settled in full, and she has not received any services from your establishment in recent months.
Upon receiving this bill, I promptly contacted your office on January 17, 2024, seeking clarification. A representative informed me that the charge was associated with a dental appointment dated January 7, 2022. I expressed my concern during this conversation, highlighting that my dental insurance coverage has been continuously active without lapse, and such services should have been fully covered.
Further investigation with my insurance provider, Moda Health, revealed that the billing discrepancy stemmed from incomplete claim information submitted by Monarch Dental. Moda Health advised that the necessary corrective action would involve Monarch Dental either providing the missing details or absolving the charges.
Despite multiple attempts to resolve this matter directly with your office on January 17, 19, 24, 30, and February 13, 2024, my efforts were met with unfulfilled promises of callbacks from management.
It was not until February 20, 2024, that I was able to discuss the issue with a manager, who informed me of a claim resubmission that was ultimately denied due to filing beyond the permissible period.
This situation has placed me in an unjust position. My insurance representative, Amy, confirmed that Monarch Dental had initially submitted the claim on January 11, 2022, but failed to include essential billing address information. The claim was sent in as two separate claims. One had a claim with a picture of an x-ray for $130.
The other was a 17-line-item claim. Both claims were for the January 7, 2022, appointment. Upon receipt, the insurance company notified Monarch that for the claim to be approved, a billing address location was required. After not receiving a timely response from a Monarch representative, Moda Health sent a subsequent notice on January 18, 2022, stating Monarch had 15 days to provide the required information.
A Moda Health representative confirmed that Monarch representatives did not respond to the request for information within the 15-day window, or there after prior to December 19, 2023, and thus the claim was deemed closed/pending on February 8, 2022.
A subsequent attempt to correct this error from Monarch was made on December 19, 2023, well outside the standard one-year filing deadline, resulting in denial due to untimeliness on December 30, 2023.
It is my position that the responsibility for this oversight should not fall to me, the patient, especially considering the administrative nature of the error and the significant delay in claim submission by Monarch Dental.
This lapse in administrative diligence has resulted in undue financial burden and stress.
Therefore, I respectfully request that Monarch Dental reviews this matter thoroughly, considering the outlined circumstances and the information provided by Moda Health. I urge you to take appropriate action to rectify this billing error by removing the unwarranted charges from my account.

Over charge customers
Had procedure done and Humana said it would be $1184 out of pocket and Monarch charged $2778. Monarch is in network with Humana that is why I went there.
Cannot get a response from Monarch except that is what the out of pocket is.
UNACCEPTABLE!!! Be wary of what Monarch tells you what the charges will be.

AWFUL, more happened than what I'm complaining about now
Basically, I'm being overcharged for an extra cavity that was marked incorrectly. I prepaid. After treatment, I asked for credit on my Care Credit account. She offered me a credit for my next cleaning (I get two free a year). On the matter of crediting my Care Credit, she said it hasn't gone through yet. All I want is to get my due money back; at this point, it would be nice to even have a correspondence regarding this matter.

Apparently they have a company policy preventing parents from being in the room with their child. My daughter was going to have a baby tooth pulled and they forced me to sit in the lobby, claiming the reason had something to do with Hippa.
I know they were lying. The worst part was Dr.
Dean had a really nasty, arrogant attitude. She was extremely unprofessional.

First Visit As a New Patient.....

I was referred to this location to see Dr. Karla Castillo, DDS.
My appointment was for 1:30 for a consultation... I arrived 30 minutes earlier. I was seen about 30 minutes later... had 4 X-rays done, seen the doctor for 1520 minutes, and it was decided I would need a root canal.
Not only that, but I waited an additional 20 min to learn of my patients' responsibility, which I was fine with.
I then sat back in the waiting room for another 1:45 minutes. I was the only one left in the waiting room. There were 4 employees sitting in the check in area and not one brother to acknowledge my present or kept me informed on the status of my appointment.
I finally went to the window and ask, "Will I still be seen?" I was then told I can go sit in the room but will still have to wait... I was there from 1:00 to 4:40...
20 minutes before closing time. I refuse to be seen when everyone including the physician is ready probably ready to go home... the procedure was too important to feel possibly rushed... Management needs to see to it that there is better communicate with a the patients.
One last thing, it may seem petty.. Please check your restroom for cleanliness.
I was referred to Monarch Dental by CULEBRA NORTH DENTAL CARE. One of the most professional, (the Staff) is so personable,
(this includes the Doctor), attentive, informative, and manages time very well. They acknowledge you as if you are the only patient they were seeing that day.
From the time you arrive to the time you leave.
All your questions are answered. The Clinic is quiet and very clean.
Billing ineptitude
While the staff at the location on Old Denton in Carrollton is lovely, they apparently have no idea how to file an insurance claim. Over the last year every time I have gone in for an appointment, which was every couple of months due to some long term issues, I have had to pay an additional $125-$200 that I am told my insurance ended up not covering.
I called my insurance, Cigna, and they said they were trying to look into it for me but the office manager was either not answering their calls, or promising to send the information needed and then not following through.
I just received a notice from my insurance company that a visit from early January, which was due to a crown falling off that had only been placed a month earlier, was denied because the dentist office failed to provide all of the billing information again, trying to say we will now owe the dentist $2000.
The office manger had gone home early and so my insurance is going to have to try to call them again tomorrow. With their history I am not holding out hope they will respond.

Incident on /23/24 around 2pm. Female employee answered.
Stated I wanted the doctor to write me a prescription for a mouth issue. I asked her to just say yes or no whether the doctor can she said no.
Then realize the doctor had previously written me a prescription for something else. When I tried to correct her, she refused to allow the conversation to be redirected and just kept repeating herself you said say yes or no.I said, whats your name she refused and hung up on me!!
