Eventually went elsewhere for more $ cost effective treatment versus excessive charges of Monarch. Over one year latter Nov 2017 get letters from Monarch telling me I owe them $$. Unbelievable.... over one year later.
I make couple of payments & stop getting monthly invoices & start getting calls from " SMILE" collectors - parent company ?? of Monarch.
Request that they send invoice am still waiting.. STAY AWAY FROM THESE jerks screwed up billing cycles/ process - they apparently do not know how to submit bills to insurance providers...
